The following are required with this application as attachments:
A) Balance sheet (may be handwritten) showing your current assets estimated at fair market values and your current and long-term liabilities, including any credit card balances, taxes owed, outstanding medical bills, etc.
B) A monthly cash flow of your income and expenses
C) The first four (4) pages of your most recent federal income tax return (Form 1040). If the return does not reflect your current situation, please explain.
